Covid-19 Vaccine Scheduling

The COVID-19 vaccine is gradually becoming more available. Northwest Eye Surgeons has now opened vaccination sites at our clinics in Seattle and Mount Vernon Clinic locations. Your health and well-being remain our highest priority. All vaccine appointments should be made online. There is no need to call our offices to schedule a vaccine appointment.

Appointments are required and availability is limited. You must make two appointments, approximately 4 weeks apart.  The number of vaccines we receive will influence how many appointments we will have. State and federal health partners determine how many vaccines we get. We will hold vaccine clinics on specific dates per location.

 Parking is free at both locations. Vaccine Clinic hours: 8:00 am – 4:30 pm

All vaccine appointments will be made online. Please do not call Northwest Eye Surgeons or our Sight Partners affiliates for vaccine appointments or updates. We need to keep our phone lines open. Any vaccine calls will be redirected to our online registration website. Instructions for the booking appointment:

  1. Enter your contact information, name, email address, phone number.  
  2. Enter your date/time for your first vaccine appointment.
  3. Enter your date/time for your second vaccine appointment. The 2nd dose available date will be indicated.

There will be no charge for the vaccine. Bring a photo ID and insurance card (if available). You may be required to show proof of age 18 yrs or older. Costs for administering the vaccine are covered by insurance, and will be billed. If a person is eligible for the vaccine and doesn’t have insurance, they can still receive the vaccine at no cost.

You will receive a date and confirmation email along with directions to the clinic. Only individual appointments can be scheduled. We can’t schedule a group appointment at this time. Please plan to fill out the personal health intake form for each 1st and 2nd vaccine appointment. You can download and complete the form before your appointment or print it out to bring with you.
